On or about July 27, 2020, An employee was exposed to fall hazards on a commercial construction site in Urbana, Illinois, and adequate fall protection was not being used when walking/working surfaces 6 feet or more above a lower level.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee On or about July 27, 2020, An employee was exposed to fall and crush-by hazards on a commercial construction site in Urbana, Illinois, when anchoring off of an aerial lift and working outside the basket because the lift is not designed for a fall protection anchorage point. In the event the employee would fall would apply an eccentric load, and the unit could tip as a result of the eccentric load.
