29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about July 29, 2020, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 1500 Marlowe, Lakewood, Ohio 44107. The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about July 29, 2020, employees were exposed to a fall of approximately 22 feet while performing roofing activities on a residential home located at 1500 Marlowe, Lakewood, Ohio 44107. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection. Santos Lopez Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1335321, citation number 1, item number 1 and was affirmed as a final order on 8/22/2018, with respect to a workplace located at 1488 W 116th, Cleveland, Ohio 44102.
