29 CFR 1926.20(b)(4):The employer did not ensure that the employee(s) required to operate equipment and machinery was qualified by training or experience. (a) Connell Resources, Inc. at 4th Street and 26th Ave in Greeley, CO 80536: On and before July 31, 2020 the employer did not ensure employees were instructed in the recognition and avoidance of unsafe conditions while operating demo saw. Employees were not effectively instructed nor were procedures implemented for the avoidance of kick-back during operation of demo saw by using a jack stand to support the pipe while being cut. This condition exposed employees to struck-by hazards when operating the demo saw. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
