29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) Connell Resources, Inc. at 21st St SW & Crystal Ct., Loveland, CO: On and around January 22, 2021, the exposing employer did not instruct each employee in methods of communication between equipment operators and workers to avoid unsafe conditions or eliminate hazards or exposure to injury. This condition exposed employees to struck-by hazards and caught-between hazards and resulted in an employee being hospitalized after being struck in the head by a front-end loader bucket, while removing frozen aggregate from a sidewalk.
