29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Basement restroom: Employees were installing wall and floor tile. The following equipment was identified missing the grounding pin on their flexible cord: a) A Husky task light (Model #K40069) was used to illuminate the workspace in the restroom while employee(s) installed tile along the wall. b) The Tower Mfg. Corp GFCI adapter (Catalog No. 30376), which was connected to the DeWalt powered wet tile saw, was used to cut corner pieces for the floor. Employees were exposed to electrical shock hazards using equipment without a grounding pin in areas where water was present.
