29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: At the jobsite located at 399 Silver Dollar City Parkway, Branson, Missouri 65616, employees were exposed to a serious fall hazard. Employees accessing a water pump to conduct work via a rock wall formation were not protected from falling by the use of guardrail systems, safety net systems, or personal arrest systems. Employees were exposed to a fall hazard of between 5 to 12 feet to the ground below.

The horn on the CASE 586H forklift was found to be defective. The forklift had not been taken out of service until restored to safe operating condition.
