On August 6, 2020 at 150 Heritage Lane in Oshkosh, Wisconsin, an employee was rigging materials using two two-leg chain slings on a boom truck crane without utilizing any head protection.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a) On August 6, 2020 at 150 Heritage Lane in Oshkosh, Wisconsin, an employee was hand signaling a crane operator from a third floor at an unprotected edge twenty feet above surface level without utilizing fall protection equipment. b) On August 6, 2020 at 150 Heritage Lane in Oshkosh, Wisconsin, employees were installing interior walls on a third floor with an unprotected edge twenty feet above surface level without utilizing fall protection equipment.
