29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  (a) On or about August 11, 2020, employees working on the low pitch area of the roof located over the front porch of the structure, did not use eye protection when using pneumatic nail gun and leaf blower. This exposed employees to eye foreign body/impalement hazard from nails and construction debris being blown off the roof.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a)  On or about August 11,2020, the employer did not keep a written verification that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential roofing activities, 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  (a) Employees conducting roofing work on a 2:12pitch, and a 8:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately eleven (11) feet, 4 (four) inches.  The A&J Roofing Solutions company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1449259, citation number 1, item number 2 and was affirmed as a final order on December 19th, 2019, with respect to a workplace located at 449 Randolph Street, Burlington, Wisconsin.
