29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o); On or about August 6, 2020 the employer failed to ensure that the belt and pulley system providing power to the flywheel of the line 8 cut off press was effectively guarded to prevent employees from coming into contact with the ingoing nip point between the pulley and belt.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 12, 2020 the employer failed to ensure that effective machine guarding was in place on the line 8 hydraulic cut off press. Employees were exposed to the hazard of the closing of the ram area and moving table parts within the equipment. An administrative control in the form of a chain rope and warning signage was placed in front of the hazard area.
