This was most recently documented at 5614 Highland Drive, Papillion, Nebraska on August 11, 2020 when employees were installing floor sheathing on the 2nd story of a residential structure, in close proximity to an unguarded elevator shaft, and no fall protection was being used.    On August 11, 2020, an employee received severe injuries when he fell through the unguarded elevator shaft opening 25 feet to the basement floor.
