29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: The employer is failing to protect employees from fall hazards while performing residential framing activates. This was most recently documented at 5614 Highland Drive, Papillion, Nebraska on August 11, 2020 when employees were installing floor sheathing on the 2nd story of a residential structure, in close proximity to an unguarded elevator shaft, and no fall protection was being used. On August 11, 2020, an employee received severe injuries when he fell through the unguarded elevator shaft opening 25 feet to the basement floor.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose: The employer is failing to protect employees from electrical hazards when using electrical equipment in a temporary application that is intended for permanent installations. Employees were using a portable extension cord with partially stamped access knockouts on its junction box. All knockouts were installed.
