29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure. On or about May 8, 2025, three employees were engaged in residential framing activities, located at 10410 S 166th Circle, Omaha, NE 68046, were exposed to fall hazards in that the employees were working in excess of 6-feet while conducting framing operations without fall protection.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: On or about May 8, 2025, three employees were engaged in residential framing activities, located at 10410 S 166th Circle, Omaha, NE 68046, were exposed to fall hazards in that the employees were using the top two steps of the 8-foot "A-frame" ladder to access the floor joists. The joists were approximately 9 feet 11 inches above the lower walking/working surface requiring the employee to step up an additional 1 foot 11 inches, exposing them to increased potential fall risk.
