29 CFR  1926.404(a)(2):A grounded conductor was attached to a terminal or lead so as to reverse designated polarity:  a.   On or about August 12, 2020 at times prior, carpentry employees using 115 Volt AC power tools at the job site were exposed to an electric shock hazard and to laceration or amputation hazards in that the extension cord used by employees from the temporary power post outlets to the power strip was missing the grounding prong, and the plug polarity was reversed at the outlet.

29 CFR  1926.404(b)(1)(iii)(C):Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  a.    On or about August 12, 2020, and at times prior, employees were exposed to 115 Volt electric shock hazards in that extension cords used at the job site were not visually inspected each day, and one of the extension cords was missing a grounding prong and had damaged insulation.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   On or about August 12, 2020, and at times prior, on the top cords of the open trusses, two framing employees were exposed to a 20 feet fall hazard down through the truss framing in that the were not protected by a fall protection system.  b.   On or about August 12, 2020, and at times prior, at the edge of the second floor at the open stairwell, employees were exposed to a 10 feet fall hazard in that there was no guard rail system, nets, or use pf personal fall protection equipment to protect employees from the fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.   On or about August 12, 2020, and at times prior, on the job site at the kitchen, the side rails of the aluminum extension ladder used for access to the second floor did not extend at least three feet above the upper landing surface, exposing employees to an 11 feet fall hazard.

On or about August 12, 2020, on the job site, at the kitchen, the aluminum extension ladder used for access to the second-floor was placed with the ladder feet on loose scraps of framing wood that could allow the ladder feet to displace and allow the ladder to fall, exposing employees to an 11 feet fall hazard.

On or about August 12, 2020 and at times prior, at the left side of the front entrance, a framing employee installing hip truss pieces to the porch roof stood on the top and top step of the 6-foot Werner stepladder.  b.   On or about August 12, 2020 and at times prior, at the right side of the front entrance, a framing employee installing hip truss pieces to the porch roof stood on the top and top step of the 6-foot Werner stepladder.
