OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer failed to maintain underhung cranes in a safe condition, employees operated underhung cranes with broken, damaged or missing parts and/or missing or bypassed safety devices and were exposed to crushing injuries from uncontrolled loads lifted by the underhung cranes and hoists:      UNDERHUNG CRANES: The employer did not ensure that broken, damaged or missing parts and/or missing or bypassed safety devices were repaired before allowing employees to operate under hung cranes.  Employees used these cranes to move sheets of metal and were exposed to crushing hazards, observed on or about August 12, 2020.  (a) H-800 - Employees moving metal plates ranging from 50 to 3,500 pounds were exposed to crushing hazards from a broken hook safety latch, on or about August 12, 2020.  (b) Rail Mazak - Employees moving metal plates ranging from 50 to 500 pounds were exposed to crushing hazards from a broken hook safety latch, on or about August 12, 2020.  (c) Hamaii - Employees moving metal plates ranging from 50 to 3,500 pounds were exposed to crushing hazards from a broken hook safety latch, on or about August 12, 2020.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation:  a) Mold Base Industries - The employer did not list object or substance that directly injured or made the person ill, in column F for each employee listed on the 300 log for 2020.  The last injury documented before inspection was on April 30th, 2020.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.184(e)(3)(ii): Records of the most recent month in which each alloy steel chain sling was thoroughly inspected were not available for examination:  a) Steel Chain Sling Inspection - The employer did not create and provide the most recent month in which each alloy steel chain sling was thoroughly inspected exposing employees to struck-by hazards, on or about August 12th, 2020.  Note: A thorough inspection of alloy steel chain slings requires (A) frequency of sling use; (B) severity of service conditions; (C) nature of lifts being made; and (D) experience gained on the service life of slings used in similar circumstances.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
