29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) On or about 08/17/2020, by the concrete form area, a 110VAC plug box used by employees was not effectively secured to its mount resulting in an opening/gap where its inner wiring was exposed to the elements. A metal wire had also been wrapped inside the exposed opening/gap and protruded out of the opening/gap. This condition exposed employees to electric shock hazards. b) On or about 08/17/2020, by the concrete form area, a 110VAC breaker box had a 7/16 inch opening next to an electrical plug exposing its inner wiring to the elements. This condition exposed employees to electrical shock hazards. c) On or about 08/17/2020, by the concrete form area, multiple 110VAC electrical boxes had unused, open knockouts on the bottom side of the boxes exposing inner wiring to the elements. This condition exposed employees to electrical shock hazards.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: a) On or about 08/17/2020, as observed at the concrete form area, conduit entering an electrical box was not fitted to the electrical box resulting in potential abrasion to the internal wiring.
