29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) DaVinci Sign Systems, Inc. at 205 W. Oak St., Denver, CO: On and around August 20, 2020, the controlling employer did not initiate a program that provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person. This condition exposed two subcontractor employees, who were not utilizing a fall protection system, to fall heights up to approximately 12.2 feet from the roof of a commercial building and 11 feet from a scaffold.

Oak St., Denver, CO: On and around August 20, 2020, the controlling and exposing employer did not ensure a written fall protection training certification record was prepared, which included the identities of employees trained, dates of training, and the signature of the trainer or employer.
