29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling:  On or about August 17, 2020, employees stood on platforms on bins 4 and 7 while observing another employee in the bin. The platforms were over 4 feet above the ground and did not have guardrails thereby exposing employees to falls.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.272(g)(4): The employer did not provide equipment for rescue operations, which was specifically suited for the bin, silo, or tank being entered:  On or about July 1, 2020 through July 31, 2020, at the above address, employees were performing work in grain bins 1 and 2. Rescue equipment was not provided while the employees were exposed to engulfment hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.272(j)(1): The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other exposed surfaces.  On or about August 19, 2020 and prior, at the above address, the employer did not implement a written housekeeping program resulting in a buildup of dust in the dump pit area exceeding 1/8 inch of dust exposing employees to a fire and explosion hazard.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
