A) 1200 State St, Perth Amboy, NJ 08861 - Buckeye Terminals Perth Amboy Employees were working inside an excavation with evidence of water. They were in the process or attaching a manhole collar to a rigging system when the collar shifted creating a gap on the ground. An employee fell inside the gap and injured his ankle and rib cage. Condition noted on or about 08/27/2020
