29 CFR 1926.502(b)(11): When guardrail systems are used at holes, they were not erected on all unprotected sides or edges of the hole. At the site - Building 1300 1st Floor Elevator Pit, employees were working installing elevator shaft doorway CMU infill with cement in front of a open hole/elevator pit, exposing employees to a 5 feet fall hazards, on or about 8/25/2020.
