29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. (a) 190 Greenhorn Drive, Pueblo CO 81004: On or about August 4, 2020, and at times prior, employees were exposed to struck by and crushing injuries while performing cleaning tasks and clearing jams due to lack of steps to control hazardous stored kinetic energy on the side arm machine. One employee experienced an amputation injury while cleaning debris from the machine while kinetic energy was not adequately controlled. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) 190 Greenhorn Drive, Pueblo CO 81004: On or about August 28, 2020, and at times prior, employees were potentially exposed to crushing and amputation hazards due to lack of point of operation guarding on the TPS machines. Guards on the TPS machines were partially removed and no longer provided adequate protection from amputation hazards. b) 190 Greenhorn Drive, Pueblo CO 81004: On or about August 28, 2020, and at times prior, employees were potentially exposed to thermal hazards due to lack of protection from flying chips generated by the TPS machines. Wire guards on the TPS machines were not adequate to protect passersby from flying chips of heated metal. c) 190 Greenhorn Drive, Pueblo CO 81004: On or about August 28, 2020, and at times prior, employees were potentially exposed to pinch points due to lack of point of operation guarding on the assembly room welding machine and crimpers. The welding machine and crimping machines use two hand controls, however the machine continues the cycle even if the controls are released, creating a pinch point hazard. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
