(a) 190 Greenhorn Drive, Pueblo CO 81004: On or about August 17, 2021, and at times prior, employees were potentially exposed to fall hazards while using a personal fall-arrest system in the heat-treat room. The retractable lanyard had a visible kink in the line and was not removed from service prior to use.
