29 CFR 1910.178(f)(1): The storage and handling of liquid fuels such as gasoline and diesel fuel shall be in accordance with NFPA Flammable and Combustible Liquids Code (NFPA No. 30-1969), which is incorporated by reference as specified in Sec. 1910.6. On or about 9/1/2020 at the portable diesel fuel refilling tank, the employer failed to protect the liquid fuel storage container from being struck by or damage by vehicles and equipment operated near the container. No abatement required

On or about 9/1/2020 at 6757 Covington Highway, Lithonia, GA. where the employer failed to ensure that electrical extension cords were not used in a "Daisy chain" configuration, exposing workers of all trades to 120 V electrical shock and burn hazards where: a. 2nd floor GFCI panel powering a 300V, yellow extension cord, powering a 300V yellow extension cord, powering a 120V Makita 5007F circular saw S/N 3276Y (Awbrey). b. 3rd floor GFCI panel powering a 300V, yellow extension cord, powering a 300V yellow 3-way extension cord, powering a Porter Cable compressor and Black and red 300V extension cord, powering a 120V Makita 5007F circular saw S/N 4547Y (Awbrey) c. 3rd floor GFCI panel powering a 300V, blue/red/white extension cord, powering a 300V yellow 3-way extension cord, powering a 300V black and yellow extension cord, powering a 120V Makita 5007F circular saw S/N 1879715Y and a 300V red and black extension cord, powering a 120V Makita 5007F circular saw S/N 1641356 (P Chico) d. 3rd floor GFCI panel powering a 300V, blue/red/white extension cord, powering a 300V yellow 3-way, powering a blue/red/white 300V extension cord powering a Worm drive SPT77W Skilsaw S/N 810002816, and a 300V yellow and black extension cord, powering a DeWalt DWE575SB circular saw S/N W0869DO.
