29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: In the Maintenance and Tire Shop, the Porter Cable bench grinder work rest was adjusted to approximately 1/4 inch from the wheel. The distance was greater in the middle, because due to wear, the work rest was no longer a flat surface, which created a larger opening in the middle of the work rest, preventing the guard from being adjusted to 1/8 inch gap across the entire length of the guard, increasing the potential for a part to become lodged against the wheel during grinding, exposing employees to a struck by and caught in hazard.
