This exposed employee(s) to a struck by and caught in hazard, that can result in cuts, lacerations, punctures, and amputation injury.

(a) On or about September 04, 2020, Injection molding machine setup persons (classified as affected employees by employer) were performing tasks classified as tasks to be preformed by an authorized employee. The affected employee(s) had not been instructed, and/or trained on the recognition of the energy source hazards associated with the equipment, or the means necessary to isolate that energy. This exposed employee(s) working on or around the injection molding machine to point of operation, ingoing nip point, rotating parts, injection hazard and amputation injury.
