29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about September 9, 2020, at an excavation site located in a field adjacent to 161st and Memorial in Bixby, OK, employees were allowed to enter and work in a dual leveled excavation with vertical/unprotected walls, exposing the employee to the hazard of being crushed by soil.
