29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about February 23, 2021, at an excavation site located in a field by the intersection of 118th and Memorial in Bixby, OK, employees did not wear protective head gear while working in a trench ranging from 8'4" to 5'6" deep, exposing the employees to struck-by and crushed-by hazards. United Utilities Specialists, L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.100(a), which was contained in OSHA inspection number 1250833, citation number 1, item number 1, and was affirmed as a final order on November 7, 2017, with a final abatement date of October 25, 2017, with respect to a workplace hazard one block west of 145th and 131st in Broken Arrow, OK 74011.

Employees are exposed to struck-by, crushed-by and fall hazards.
