29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about February 23, 2021, at an excavation site located in a field by the intersection of 118th and Memorial in Bixby, OK, employees are allowed to enter and work in a trench more than 5' in depth without protective systems in place, exposing the employees to struck-by and crushed-by hazards.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about February 23, 2021, at an excavation site located in a field by the intersection of 118th and Memorial in Bixby, OK, employees did not wear protective head gear while working in a trench ranging from 8'4" to 5'6" deep, exposing the employees to struck-by and crushed-by hazards.  United Utilities Specialists, L.L.C.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.100(a), which was contained in OSHA inspection number 1250833, citation number 1, item number 1, and was affirmed as a final order on November 7, 2017, with a final abatement date of October 25, 2017, with respect to a workplace hazard one block west of 145th and 131st in Broken Arrow, OK 74011.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about February 23, 2021, at an excavation site located in a field by the intersection of 118th and Memorial in Bixby, OK, employees are allowed to work in a trench more than 4 feet deep that had no ladder or other safe means access/egress. Employees are exposed to struck-by, crushed-by and fall hazards.  United Utilities Specialists, L.L.C.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 1250833, citation number 1, item number 2, and was affirmed as a final order on November 7, 2017, with a final abatement date of October 25, 2017, with respect to a workplace hazard one block west of 145th and 131st in Broken Arrow, OK 74011.
