Mark Erie Hospitality, LLC, 18 West 18th Street, Erie, PA 16501, on or about September 11, 2020 and times prior thereto:  Site contractors were engaged in demolition and painting operations.  A fire protection program was not developed for the site.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Mark Erie Hospitality, LLC, 18 West 18th Street, Erie, PA 16501, roof, north side of the structure, on or about September 24, 2020 and times prior thereto:  Site contractors were performing roofing activities that included, but was not limited to, removing debris.  The top rail of the guardrail system was not installed.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Mark Erie Hospitality, LLC, 18 West 18th Street, Erie, PA 16501, roof, west side of the structure, on or about September 24, 2020 and times prior thereto:  Site contractors were performing roofing activities that included, but was not limited to, removing debris.  The top rail of the guardrail system was deficient in that it was equipped with caution tape and would not meet strength requirements.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
