29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. a) Welding Shop: On or about September 2, 2020 an emergency eyewash station was blocked by a trash bin and did not provide immediate access for emergency use exposing employees to corrosives materials, such as hot slags generated from welding operations.

29 CFR 1910.252(c)(1)(ii): When welding had to be performed in a space entirely screened on all sides, the screens were not so arranged that no serious restriction of ventilation exited: a) Welding Shop: On or about November 5, 2020 an employee performing flux shielded arc welding (stick welding) on structural steel was not using a screen exposing nearby workers from arc glare and ultraviolet (UV) radiation hazards.
