29 CFR 1926.1053(a)(24): The side rails of through or sidestep fixed ladders did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder: Repair Modernization of Towers D, E, & F, Building E: Employees utilizing a fixed ladder to access the roof top through a roof hatch. A ladder extension was not installed that extended 42 inches above the roof top and employees were exposed to 12 foot fall hazards.
