29 CFR 1910.136(a): The employer did not ensure that each affected employee used protective footwear when working in areas where there was a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet were exposed to electrical hazards:     A)     Peerless Stove & Manufacturing Company, Inc., doing business as Peerless Ovens, located at 334 Harrison Street, Sandusky, OH 44870: On or about September 2, 2020, the employer did not ensure that employees were protected by adequate personal protective equipment such as, but not limited to, steel toe shoes while employees were required to work with sheet metal. There was a danger of foot injuries due to falling sheet metal in that employees were not required to use adequate foot protection.

Employees were exposed to struck-by and caught-in-between hazards that could result in serious injuries while operating the Wysong Single End Frame Mechanical Squaring Shear.

The employer or employers representative did not contact the OSHA Toledo Area Office or the 24-hours OSHA number to notify the agency of a personal injury incident, resulting in partial amputation, within 24 hours from the moment it was reported to the employer or employers representative.
