29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. At the jobsite, employees were observed on the framing ceiling joists, and roof rafters using pneumatic guns (Paslode), while other employees framed below. This condition exposed employees to overhead struck-by-hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system. At the jobsite, where employees were observed working on the ceiling joists without fall protection. This condition exposed employees to fall from heights greater than 6 feet to the concrete surface below.
