29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) 2731 Main St: On or about September 21, 2020 and times prior, employees worked form a scaffold sanding foam in preparation for installing stucco.  The employees worked from scaffold heights up to 15 feet without fall protection.    b)2731 Main St: On or about September 21 and times prior, employees worked from a scaffold sanding foam in preparation for installing stucco. The top level of the scaffold was equipped with a guardrail that only contained a top rail exposing employees to falls of 18 feet to the ground.

The scaffold level were not fully planked exposing the employee to a falls of 18 feet.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:  a) 2731 Main St: On or about September 21, 2020 and times prior employees worked from a scaffold sanding down foam in preparation of installing stucco on a commercial building. The employees were working at heights up to 18 feet above ground and the erected scaffolding did not have toeboards installed on the edges of platforms.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).      a) Front corner of building: On or about September 21, 2020 and times prior, employees were working from a scaffold with a service line and electrical drop within 3 feet of the scaffold.  The voltage on the service line and drop was rated at 120-240 volts exposing employees to electrical shock.

The employees were not utilizing a fall protection system or fall arrest harnesses.
