29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  a) Printing Press #972 - Employees working from the moveable deck of printing press #972 were not protected from stepping into a floor hole which measured a minimum 2 ¼ inches wide up to approximately 16 inches wide at the time an employee was injured while attempting to step over the hole, on or about September 12, 2020.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
