29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)LSC Communications US, LLC dba Quality Park at 100 Quality Park Drive, Beresford, SD 57004: On or about and at times prior to June 22, 2021, the employer did not ensure that employees were protected from in-going nip points due to rotating parts. Employees were operating the Winkler Dunnebier 527GSV (#12669) reel fed envelope machine with unguarded rollers. This condition exposed employees to caught-in hazards involving the in-going nip points of the rollers. (b)LSC Communications US, LLC dba Quality Park at 100 Quality Park Drive, Beresford, SD 57004: On or about and at times prior to June 22, 2021, the employer did not ensure that employees were protected from ingoing nip points due to rotating parts. Employees were operating the Winkler Dunnebier 527GSV, (#11382) reel fed envelope machine with unguarded rollers. This condition exposed employees to caught-in hazards associated with the in-going nip points of the rollers.

29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles: (a)LSC Communications US, LLC dba Quality Park at 100 Quality Park Drive, Beresford, SD 57004: On or before and at times prior to June 18, 2021, the employer did not ensure employees were protected from caught-by hazards associated with the point of operation involving the Winkler Dunnebier (#11382) envelope machine. An employee suffered a fingertip amputation while cleaning the gummer paddles with the machine in operation. The guards had been removed which exposed the paddles. This condition exposed employees to amputation hazards associated with the point of operation of the envelope machine.

Employees were utilizing a WD-40 container used for machine maintenance and cleaning which was not properly labeled to identify the contents. This condition exposed employees to chemical hazards.
