29 CFR  1926.451(f)(5): Scaffolds were moved horizontally while employees were on them although the scaffolds were not designed for such movement by a registered professional engineer:   (a)  Jobsite:  On or about September 23, 2020, an employee was moving himself while he was on a manually propelled mobile scaffold.  The scaffold tipped over and the employee was severely injured.  The employee was at the top of the manually propelled mobile scaffold (12 feet 3 inches) when it fell over.

The training shall include the following areas, as applicable:   (a)  Jobsite:   On  or about September 23, 2020,  two employees were building a manually propelled scaffold and one employee was working from the scaffolding and neither employee was trained in how to work safely when working with/on a scaffold.

(a)  Jobsite:  On or about September 23, 2020, an employee was allowed to ride on a manually propelled mobile scaffold (Baker's scaffold) while he pulled/pushed the scaffold to move.  The height of the scaffold was 12 feet 3 inches and the width of the scaffold was 2 feet 5 inches.  This scaffold fell over and the employee was severely injured.

The fall hazard was about 12 foot 3 inches to the wooden floor.  The employee was installing insulation while he was standing at the 12 foot 3 inch level of the manually propelled mobile scaffold.
