29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite: On or about April 27, 2022, an employee was not protected from falling about 9 feet 1 inch while he was foaming in cardboard baffles at the open eaves. The employee was standing on the horizontal members of trusses to do this work. The employee slipped and fell to the wooden floor.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: (a) Jobsite: On or about April 28, 2022, the employer needs to retrain its employees on when to use fall protection on construction sites. An employee was exposed to and fell about 9 feet 1 inch from the roof trusses to the wooden floor to the house that was under construction.
