29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Automatic Flux Spray Gun; On or about September 22, 2020 and at times prior to the employer allowed employees to conduct maintenance activities on the automatic flux spray gun such as but not limited to cleaning of the nozzle while only relying on circuitry control (Open Door button) exposing employees to hazards associated with the inadvertent startup of machinery with multiple energy sources such as electric and pneumatic.    b) Core Assembly; On or about September 22, 2020 and at times prior to the employer allowed employees to conduct maintenance activities on the core assembly line such as but not limited to jam clearing while only relying on circuitry control (light curtains/keyed stop button) exposing employees to hazards associated with the inadvertent startup of machinery.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Big stamping machine; On or about September 22, 2020 and at times prior to the employer allowed employees to conduct maintenance activities on the big stamping machine without developing and/or utilizing written machine specific procedures exposing the employees to hazards associated with the control of hazardous energy.  Korens USA, Inc was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1425831, citation number 2, item number 1 and was affirmed as a final order on March 6, 2020, with respect to a workplace located at 600 Joseph St, Alexander City, AL.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days  of receiving information that a recordable injury or illness has occurred:  a) Recordkeeping; On or about June 3, 2020 the employer failed to record an accident that required an employee to be transferred to the hospital via ambulance on the OSHA 300 logs within 7 days of the accident occurring.     Korens USA was previously cited for a violation of this Occupational Safety and Health Standard 1904.29(b)(3), which was contained in OSHA inspection number 1425831, citation number 3, item number 1, and was affirmed as a final order on March 6, 2020 with respect to a workplace located at 600 Joseph St, Alexander City, AL 35010.
