29 CFR 1910.147(a)(2)(ii)(B):29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation. a) Production Floor, Central Mill Area: On or about September 28, 2020, and at times prior, employees were exposed to amputation hazards, in that the employer did not ensure that machine operators use control of hazardous energy procedures when changing press dies on equipment including, but not limited to, the Bruno 75-11 hydraulic press and the Freeman 75-3 hydraulic press.
