29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  On or about 9/28/20, the employer failed to ensure that employees accessing the storage loft in the shop were provided and utilized fall protection. The fall height was approximately  7'2" (seven feet and two inches).

29 CFR 1910.30(a)(1): The employer did not provide training for each employee who used personal fall protection systems or who was required to be trained as specified elsewhere in this subpart before the employees were exposed to a fall hazard:  On or about 9/28/20, the employer failed to ensure that employees being exposed to fall hazards were provided training, by a competent person, on how to recognize fall hazards, the procedures to be followed to minimize fall hazards, and the correct use of fall protection systems and equipment. Employees were exposed to fall hazards while accessing the storage loft and working at heights in the bucket truck.

29 CFR  1910.180(h)(3)(iii)(b):  During hoisting, care was not taken to ensure that the load did not contact any obstructions:  On or about 9/24/2020, during tree felling operations, located at 1214 12th St. NE in Canton, Ohio, the employer failed to ensure that the tree being removed from the job site did not contact any obstructions while it was being hoisted and lowered.  A limb of the tree being felled contacted an adjacent tree causing the limb to break off exposing employees in the area to struck-by hazards.
