NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a- 125 Central Avenue, Woodbury Heights NJ: On or about 09/30/20, a company employee was exposed to struck by hazards when working in a trench with side walls that were neither sloped, properly benched, shored, or shielded. This trench had a depth that ranged from approximately 6 to 7 feet deep, a width that ranged from approximately 5 to in excess of 7 feet, and was approximately 23 feet long.
