29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about 10/01/2020, at the Production area, Florida Bottling, Inc, 1035 NW 21st Ter. Miami FL 33127. An energy control program was not developed and/or implemented to protect employees from amputation hazards when performing servicing and/or maintenance on the Depalletizer Xodus, Serial No. 418912314 and/or High Level Case Palletizer, Serial No. 4189123114.

Miami FL 33127. An employee was exposed to caught-in and struck-by hazards while working in close proximity to the dunnage area of a Depalletizer (i.e. Xodus, Serial No. 418912314) with a bypassed light curtain.
