On or about 10/1/2020, while preforming roofing activity on a low slope commercial building located at 5170 Hudson Dr. Hudson, Ohio 44236, a trained employee was performing the function of a safety monitor, as part of a fall protection system. The employee was working alongside of other employees and did not ensure that a warning line was properly in place and exposed workers to a fall hazard. The retention of required skills to perform this function was not demonstrated. The employer was on the site and did not ensure that the employees demonstrated the proper use of their training, exposing them to a fall hazard.
