29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide. On or about October 01, 2020, the employer did not ensure that employees had a safe walking/working surface to work from when working from a scissor lift. Th employee worked from the guardrails of the scissor lift and was exposed to a fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 01, 2020, the employer did not ensure that an employee working from the guardrail of a scissor lift was in use of fall protection. This exposed the employee to a fall hazard.
