Guardrail systems were not in place and/or incomplete at the second and third level balconies and on lower roof levels where subcontractor employees were performing work activities.   b)  Broadview Place: On or about October 2, 2020, and at times prior thereto the controlling employer, Perryman Construction Management, Inc.,  did not ensure that roofing sub-contractor(s) workers were protected from fall hazards while performing roofing activities on a three story building.
