29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about October 5, 2020, employees, including maintenance employees, were not provided effective information and training on the hazardous nature of the combustible dusts in their work area at the time of initial assignment. a) Employees were not provided information on the operations in their work area where combustible dust is present, including dust collectors. b) Employees were not provided training that included the methods and observations used to detect the presence or release of a combustible dust in their work area, including the generation and presence of visible dust clouds, hazardous dust accumulation and signs of a combustible dust fire/smoldering. c) Employees were not provided training that included the combustible dust hazards such as elements of a combustible dust fire, deflagration, explosion, ignition sources, hazardous outcomes, hazard mechanisms and potential injuries. d) Employees were not provided training that included the measures employees can take to protect themselves from combustible dust hazards, such as housekeeping, explosion control methodologies, evacuation procedures, emergency shutdown procedures, and flame retardant clothing requirements.
