29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about October 1, 2020 at the job site located at 1344 Tyler Miller Ln NE, Massillon, Ohio 44646; during the installation of roofing material on a residential roof, the employer did not ensure that employees utilized a 24 foot aluminum extension ladder as it was designed for use. Employees utilized the extension ladder by separating the sections and utilizing the fly section without anti slip feet on a roof to gain access to the roofs working surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 1, 2020, during the installation of roofing material at a job site located at 1344 Tyler Mill Ln NE, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a multiple family home were protected from a fall of approximately 7.7 feet by utilizing a fall protection system. The Ramos Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(11) Duty to have fall protection), which was contained in OSHA inspection number 1366810, citation number 01, item number 02 and was affirmed as a final order on 3/7/2019, with respect to a workplace located at 1133 10TH St NE, Massillon, Ohio 44646.
