29 CFR 1910.243(e)(1)(i): Power lawnmowers of the walk-behind, riding-rotary, and reel power lawnmowers were not guarded in accordance with the machine guarding requirements in 29 CFR 1910.212, General requirements for all machines:  a) On or about October 6, 2020 on the John Deere tractor, the HX15 Flex-Wing the power take-off shafts were unguarded from contact exposing employees to rotational caught-in hazards and the employer did not ensure spinning shaft guarding was installed.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  a) On or about September 7, 2020 in the maintenance garage a metal junction box was used on the end of a flexible electrical cable to power a 240 volt welder exposing employees to electrical shock hazards and the employer did not ensure electrical equipment was installed and used in accordance with any instructions including in the listing and labeling.

29 CFR 1910.334(a)(2)(ii): If there was a defect or evidence of damage that might have exposed an employee to injury, the defective or damaged item was not removed from service, and an employee was able to use it until repairs and tests necessary to render the equipment safe had been made:  a) On or about September 7, 2020 in the maintenance garage the Millermatic welder had a damaged section of outer insulation at the plug head exposing employees to electrical hazards and the employer did not remove the welder from service until repairs had been made.

29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the equipment grounding conductor at the point where plugs were attached to receptacles:  a) On or about July 1, 2020 in the maintenance garage the Patton floor air circulation fan (Model: 1010MP, StyleSFE-74SP) was missing the grounding pin conductor from the plug exposing employees to electrical hazards and the employer did not ensure plugs were connected in a manner to provide continuity of the equipment grounding at the point where the plug was attached to the receptacle.

More frequent inspections were not conducted in all workplaces where there was an increased risk of accident, injury, or illness due to the nature of the work performed. Sufficient unannounced inspections and unannounced follow-up inspections were not conducted by the agency to ensure the identification and abatement of hazardous conditions:  a) On or about October 6, 2020 at the facility safety and health hazards were found exposing employees to electrical, fire, and amputation hazards and the employer did not ensure inspections were being completed and inspections were frequent enough where there was an increased risk of accident, injury, or illness due to the nature of the work performed.
