29 CFR  1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:   a) On or about October 21, 2020 at the settlement maintenance fuel storage building employees transferring gasoline and diesel fuel from a Northern Tool (Model: FC-30GC-N, Serial: 22262) and HMC Industries (Model: GC-30) Portable Container for Flammable and Combustible Liquids were exposed to fire hazards including severe burns and the employer did not ensure all sources of fire ignition were controlled prior to fuel transfer operations.

29 CFR  1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) On or about October 21, 2020 employees were exposed to struck-by laceration hazards when using the Baldor benchtop grinder (Serial: X1306267544) in the mower maintenance shop and the employer did not ensure work rests were adjusted to within one eighth inch from the wheel periphery.  Notice 2#, Item 2#, 29 CFR 1910.215(a)(4) as required by 29 CFR 1960.8(b): The employer failed to ensure work rests were adjusted to one eighth inch of the wheel periphery. The employer was cited for a violation of the same standard and same hazard on September 30, 2019, Inspection #1407144, date October 29, 2019 of final order, and means of abatement was to adjust the work rest to within one eighth inch of wheel periphery.  Notice 2#, Item 1A#, 29 CFR 1910.215(a)(4) as required by 29 CFR 1960.8(b): The employer failed to ensure work rests were adjusted to one eighth inch of the wheel periphery. The employer was cited for a violation of the same standard and same hazard on September 30, 2019, Inspection #1407799, date October 31, 2019 of final order, and means of abatement was to adjust the work rest to within one eighth inch of wheel periphery.

29 CFR  1910.1200(f)(6)(ii):  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical   a) On or about October 20, 2020 the spray bottle in the settlement maintenance break room containing Pine Oil 60% disinfectant was not labeled, not allowing employees access to general information about the hazards of the chemicals in the container, and the employer did not ensure labels were placed on containers of hazardous chemicals.  Notice 1#, Item 3B#, 29 CFR 1910.1200(f)(6)(ii) as required by 29 CFR 1960.8(b): The employer failed to ensure containers of hazardous chemicals were labelled. The employer was cited for a violation of the same standard and same hazard on May 16, 2018, Inspection #1313397, date June 12, 2018 of final order, and means of abatement was to label containers of hazardous chemicals with the product identifier and words, pictures, symbols.
