Site- On or about October 5, 2020 an employee climbing a Werner 24 foot fiberglass extension ladder that was mounted inside of an elevated SkyJack SJIII 3219 scissor lift, not rated for outdoor use, was not protected for a fall of approximately 27 feet where the employer did not ensure that a competent person erected the scaffold prior to use.
