29 CFR 1910.147(a)(2)(ii)(B): (A) An employee was required to place a part of his or her body into an area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle. A blow mold process technician employee conducting maintenance activities on Airlock #65 was exposed to caught-in and amputation hazard, in that Grinder #65 and Airlock #65 was not de-energized prior to reaching into the Airlock #65 to clear re-grind material.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Employee engaged in maintenance activities on Airlock #65 was exposed to caught-in and amputation hazard, in that no guard was utilized.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. Employees working at machine #65 cooling table, were exposed to electrical shock hazard, in that the conduit had broken exposing wires that powered the cooling fans.
