29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy and the means to enforce compliance:  On or about October 07, 2020, and at times prior thereto, at a workplace located at 601 NE 29th Street, Fort Worth, Texas; the employer did not adequately develop and implement machine specific procedural steps for all machines/equipment that used electrical, pneumatic, hydraulic, and mechanical energy sources for flashing machine operators exposing employees to the hazard of amputations, crushed body parts, broken bones, and severe cuts from unexpected energizing, start up or release of stored energy.
